This tunnel is called the cubital tunnel; its floor and walls are formed by bones, and its roof by a firm band of connective tissue. When you place your hand on the bony prominence on the inner side of your elbow, you can often feel the ulnar nerve passing there with your fingertips. The “electric shock” sensation we feel when we bump this area colloquially also comes from this nerve. Cubital tunnel syndrome generally does not begin loudly; on the contrary, it progresses insidiously and slowly. Patients often do not remember exactly when the complaints began. The main symptoms are:

  • Numbness and tingling in the little and ring fingers: This is the most typical symptom. It may also be felt on the inner part of the hand.
  • Pain on the inner surface of the elbow: Often accompanied by pain and numbness radiating to the fingers.
  • Symptoms increasing when the elbow is bent: Complaints become more pronounced while talking on the phone, driving, or reading a book.

  • The elbow remaining bent for long periods: This is the most common cause. Sleeping on the arm or keeping the elbow bent all day strains the nerve.

  • Tinel’s sign: When the ulnar nerve at the elbow is lightly tapped with a finger, an electric-shock sensation radiating to the little and ring fingers occurs.
  • Elbow flexion test: Keeping the wrist straight, the elbow is bent as far as possible and held in that position for a while; during this, numbness or tingling appears in the 4th and 5th fingers.

The most valuable method for clarifying the diagnosis and measuring the degree of nerve damage is EMG (electromyography / nerve conduction study). This examination shows where and how much the nerve is compressed. Imaging such as X-ray, and MRI when necessary, may also be requested to evaluate the elbow. The Difference Between Cubital Tunnel Syndrome and Carpal Tunnel Syndrome

Carpal tunnel syndrome, on the other hand, is compression of the median nerve at the wrist, and symptoms are felt in the thumb, index, and middle fingers. The most important first step of treatment is to eliminate the factor causing the compression. The approach is determined according to the stage of the condition and, in particular, whether or not muscle loss is present.

Non-Surgical (Conservative) Treatment

In early and mild cases where there is no muscle loss, non-surgical methods are applied first:

  • Behavior and position adjustment: Avoiding keeping the elbow bent for long periods, sleeping on the arm, and movements that put pressure on the elbow.
  • Night splint: A splint that keeps the elbow in a straight position during sleep reduces the load on the nerve and eases nighttime complaints.
  • Medication: Anti-inflammatory painkillers (NSAIDs) deemed appropriate by the doctor reduce pain and irritation.
  • Physical therapy and exercise: Nerve gliding exercises, manual therapy, and appropriate physical therapy methods help relieve the nerve.

Surgical Treatment

Surgery comes into consideration in patients who do not respond to conservative treatment, who have developed marked muscle weakness or muscle wasting, and whose complaints have become unbearable. In this case, surgery is necessary to prevent the progression of neurological damage. The main goal in surgery is to release the ulnar nerve and relieve the pressure on it; this can be done by widening the tunnel or by moving the nerve to the front of the elbow (transposition). A very good response is generally obtained from surgery. After the operation, exercises that preserve muscle strength and joint mobility are recommended under the guidance of a physiotherapist.
